A nurse will go to your house and administer your first injection and you will receive 3 extra injections to take weekly or per provider order.

This is required for everyone’s first appointment and any appointment after 6 months. You are also required to complete a telehealth appointment if the provider indicates you require further follow up due to your medical history. This will be relayed to you after your first telehealth appointment.

We serve locations within a 20-mile radius of Worcester, MA. We also cover all of Central Massachusetts and Boston, but travel beyond 20 miles incurs additional charges. Excess Mileage Fee: A surcharge of $0.59 per mile applies for travel beyond the 20-mile radius. This fee will be charged directly to the client.
We are aware of the ongoing COVID situation. So in effort to compensate our nurses for going into a home, office or hotel we charge $50 surcharge to cover their PPE expenses and added time to decontaminate all our equipment.
The Patient understands and agrees that a $100 holiday fee will be applied above the normal service fee to all appointments scheduled on the following major holidays:

Independence Day (Fourth of July)
Labor Day
Columbus Day
Thanksgiving Day
Christmas Eve (after 4:00 PM)
Christmas Day
To fairly account for heavy traffic and long-distance driving, nurses will be compensated at $75/hour for extended travel time beyond the standard service range.
This charge will also be billed directly to the client.
